  • Maintains tracking log of records requests as required by HIPAA
  • Responds to referrals promptly
  • Requests medical records (H&P, Discharge Summaries, Physician Progress notes, recent lab work, x-rays, tests, etc.) immediately upon receipt of referral to support documentation for hospice admission.
  • Follows up daily until records are received.
  • Requests additional medical records for current patients
  • Faxes medical records on transfers
  • Attaches medical records, labs, DNRs, Chest x-rays, and any patient information into the EMR
  • Requests medical records to support documentation for hospice diagnosis (ICD-10) and to determine Medicare eligibility, request prior authorizations, verification of hospice benefits, and verification of care
  • Communicates with other hospice entities for verification of inquiries and benefits.
  • Maintains new patient and current patient registration by creating MR numbers and
  • Promptly enters new and pending referrals into the HCHB system for maximization of team admissions
  • Manages new referral sources adding information into systems as needed
  • Participates in data requests for the billing department, chart audit retrieval for clinical review, and all other requests ROLE REQUIREMENTS
    • Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice, or specialty care setting
      • Demonstrated ability to triage and manage work assignments. SKILLS/ABILITIES/KNOWLEDGE:
        • Self-directed and able to work with minimal supervision
        • Proficient in accessing and utilizing the organization's EMR
        • Excellent written, verbal, customer service, and problem-solving skills
        • Excellent organizational and communication skills
        • Proficient customer-focused service skills, patient-centered, positive problem-solving advocate
