Responsibilities

The Advance Care Planning Facilitator is an active member of the Advance Care Planning program at Wellstar Health System performing the core functions surrounding advance care planning, e.g., consultation, education and evaluation. This individual is responsible for meeting with identified patients to participate in advance care planning. The individual will also be intimately involved in educating providers, patients, families and our community on the advance care planning. The position reports directly to the manager for advance care planning and has an essential role in the program.
