AccentCare is seeking a dedicated Physical Therapist to join our Home Health team, specifically for weekend shifts. As a Physical Therapist at AccentCare, you will provide in-home therapeutic care to our clients, helping them achieve optimal mobility and contributing to their overall recovery and quality of life. This role requires a compassionate professional who thrives in an autonomous environment, taking the initiative to creatively solve problems and advocate for the best outcomes for our patients. Conduct patient assessments to determine their needs and establish therapeutic care plans. Implement treatment plans using techniques to alleviate discomfort and restore function. Regularly evaluate patient progress and adjust treatments accordingly. Educate patients and families on treatments, rehabilitation techniques, and preventive measures. Coordinate with a multidisciplinary team to provide the best care and optimize patient outcomes. Maintain comprehensive clinical documentation of treatments and outcomes. Proven experience in physical therapy techniques, patient assessment, and treatment planning. Strong analytical and problem-solving skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Proficiency in clinical documentation and reporting software. Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapist license in the state of practice. Valid driver’s license and reliable transportation. CPR certified.
