Job Description:
Job Description Job Description The Case Manager provides
comprehensive oversight and coordination of services for adults
with Intellectual and/or Developmental Disabilities (I/DD)
receiving Medicaid Home and Community-Based Services (HCBS). This
role ensures services are delivered in compliance with state waiver
requirements, accreditation standards (including CQL and EAGLE),
and organizational policies, while maintaining a holistic,
person-centered approach that promotes health, safety, dignity,
independence, and quality of life. The Case Manager is responsible
for ensuring that Individual Service Plans (ISPs), Behavior Support
Plans, and Crisis Plans are implemented effectively; that services
align with person-centered and outcomes-based practices; and that
documentation and service delivery meet regulatory and
accreditation standards, as well as other duties as assigned.
