Job Description LanceSoft is seeking a local contract nurse RN Case Management for a local contract nursing job in Atlanta, Georgia. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 03/02/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Local Contract Case Manager – Critical Need (RN) Atlanta, GA Kaiser Foundation Health Plan – Regional Office (9 Piedmont Center) Schedule: Monday–Friday | 8:30 AM – 5:00 PM ? 40 hrs/week | 8-hr shifts ? Requirements - Active RN license (Compact or Georgia license preferred) - BLS certification (AHA) - Minimum 2 years of RN experience - Strong understanding of case management, care coordination, and utilization review principles - Experience working in a managed care, hospital, or large healthcare system - Proficiency with electronic medical records and case management platforms - Excellent communication, organization, and critical-thinking skills Key Responsibilities - Coordinate patient care across the continuum, including inpatient, outpatient, post-acute, and community services - Perform utilization review to ensure appropriate level of care, length of stay, and resource utilization - Develop, implement, and evaluate individualized care plans in collaboration with physicians and interdisciplinary teams - Lead discharge planning and transitions of care to promote safe, timely discharges and reduce readmissions - Communicate effectively with patients, families, providers, and external agencies regarding care plans - Ensure compliance with Kaiser Permanente policies, regulatory requirements, and evidence-based guidelines - Document all case management activities accurately in electronic systems - Participate in quality improvement initiatives and care coordination meetings About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
