dynamic Clinical Pharmacist role blending both clinical and
staffing responsibilities in a healthcare setting. The position
focuses approximately 60% of the time on clinical duties and 40% on
staffing activities, primarily serving patients within a population
health and ambulatory care framework. The core objective is to
manage medication regimens for targeted disease states, such as
diabetes, collaborating closely with patients throughout the health
system. Key Responsibilities: Provide clinical pharmacy services
with an emphasis on population health and ambulatory care Manage
patient medication regimens, focusing on chronic disease states
like diabetes Perform staffing functions including dispensing and
medication counseling as part of day-to-day operations Collaborate
with interdisciplinary teams to optimize patient outcomes Potential
to transition into a 100% clinical role as clinical programs expand
Qualifications & Desired Experience: Active Pennsylvania pharmacist
license required Experience in ambulatory care or inpatient
pharmacy practice preferred Completion of residency program
strongly preferred Proficient with electronic health records,
familiarity with Meditech system advantageous Demonstrated passion
for patient-centered care and clinical program development
Location: Based in State College, Pennsylvania, with a hybrid work
model balancing clinical and staffing duties Schedule: Variable
days and afternoons, predominantly day shifts Eight-hour shifts
