1/26/2026 About the Position We are seeking an experienced RN Case
Manager with a strong background in acute care to coordinate and
manage patient care throughout the hospitalization continuum. The
RN Case Manager will assess patient needs, develop and implement
individualized care plans, coordinate with physicians and
interdisciplinary teams, facilitate discharge planning and
transitions of care, and ensure compliance with clinical,
regulatory, and payer requirements. This role focuses on optimizing
patient outcomes, reducing length of stay, preventing readmissions,
and ensuring safe, efficient, and cost-effective care delivery in
an acute care setting. Requirements Requirements: Active RN license
(state or compact) Minimum 3–5 years of acute care nursing
experience Experience in case management, utilization review, or
discharge planning Strong knowledge of acute care workflows and
care coordination Excellent communication, organization, and
critical-thinking skills EMR/EHR documentation experience Case
