of Hospice experience License: Must have a valid nursing license in

HomeBase and Oasis - Recent home health or hospice experience
Description: Provides and coordinates the provision of home nursing
care according to physician's orders. Responsible for the competent
application of the nursing process based on agency policies and
procedures. Directly supervises and evaluates Home Health Aides and
Licensed Practical Nurses. Home visits are required, along with
periodic office meetings. Reliable transportation is necessary.
Current driver's license and liability insurance are required.
Average number of patients per day is 5-10, which equates to
roughly 6-8 points. Types of cases include start of care, wound
care, and follow-up visits. Candidate must be willing to travel up
to 75 miles from the designated care center. Mileage is reimbursed
at the IRS standard rate. Documentation must be completed within 24
hours of the patient appointment. All documentation must meet
billing and follow-up patient care standards. Expectations include
accepting every patient on the schedule, regardless of distance.
The candidate must be comfortable with potentially long drives
between patients. No guaranteed hours exist with this contract.
Weekly productivity points must be met. On-call duties may be
