Responsibilities: Patient Assessment Collaborate with physicians to
establish and evaluate treatment programs, care plans, goals, and
discharge plans tailored to patient needs. Develop individualized
plans of care in conjunction with patients and their families.
Create realistic discharge plans that incorporate patient and
family goals. Treatment Planning and Provision of Care Implement
physical therapy treatment plans effectively. Select appropriate
activities to help patients progress toward their goals. Revise
treatment goals and plans based on ongoing patient and family
input. Provide continuous education and training for patients and
families. Offer discharge instructions, follow-up, and referrals to
community resources as needed. Serve as a resource for staff,
families, and physicians regarding rehabilitation services and
equipment. Documentation Document evaluations and care plans
according to departmental guidelines. Record each treatment session
in line with established protocols. Complete all required
documentation, including family education and discharge summaries.
General Requirements Uphold ethical standards, demonstrating
integrity and honesty. Embrace diversity and foster teamwork.
Always maintain patient confidentiality. Adhere to attendance and
punctuality policies. Follow departmental dress code and display ID
badges appropriately. Communicate effectively with team members,
both verbally and in writing. Participate in staff meetings,
required in-services, and process improvement activities. Organize
workload efficiently and utilize materials safely and
cost-effectively. Observe infection prevention precautions.
Customer Service Lead with vision, innovation, and a commitment to

competencies as required by the department. Maintain licensure
through ongoing education. Participate in the orientation and
training of new employees. Fulfill all mandatory hospital and
system requirements. Required Qualifications: Education: Doctorate
in Physical Therapy required. Licenses and Certifications: Physical
Therapist license required. Basic Life Support (BLS) certification
from AHA required. Experience: Relevant experience in physical
therapy required. Skills: Strong communication skills. Customer
service orientation. Team collaboration abilities. Basic computer
