  • MUST have Home Health experience
  • Must have HCHB/OASIS experience
  • Oasis E - after Jan 2023 experience is required
  • No more than 1 year gap
  • All documentation due same day
  • Homecare Homebase (Productivity points, minimum average 26-28)
  • That could mean 3 people or 7 people
  • About 20-25 cases per week. 5-7points per day
  • Absolute must - true home health experience, HCHB/Oasis documentation Overview:

    Other Details:
    Once a week they have to go into the office (Usually Tue, Wed and Thur). Mandatory in person
    Hours: Branches are open 8am - 5pm, they may have patients who want to be seen before or after.
    Homecare Homebase MUST be completed at the patient's home.
    Absolute must - true home health experience, HCHB/Oasis documentation
