Job Description:
Responsible for assessment of all psychosocial factors and
participation in the treatment planning of patient's families.
Education
Master's Degree
Experience
One year of related experience. Requires working knowledge of
standard practices and procedures.
Licenses & Certifications
*Driver's License
*Vehicle Insurance
*BLS or must be obtained within 30 days of hire.
Qualifications
*Degree in social work and currently licensed or pursuing licensure
as a social worker in the state of Georgia preferred.
*Master's degree preferred from a field related to social work with
one (1) year of social work experience acceptable.
*One (1) year of home health experience preferred.
