We are proud to have become a shining example of what's possible when the brightest professionals dedicate themselves to making a difference in the healthcare industry, and in people's lives.ResponsibilitiesWellStar Health SystemsRN Hospice AdmissionFull-TimeFriday, Saturday, SundayThe Registered Nurse (RN) - -Hospice Admission -is a licensed professional nurse who educates the patient and family regarding hospice services and the palliative approach -to terminal care so that they are prepared to make an informed decision regarding agreement for hospice admission, performs the admission for the hospice patient choosing to live at their home, ALF or SNF. The admission process requires a nursing assessment and determination if the history of the disease and the patients current condition support the hospice eligibility, The admission RN reports the findings, assesses the needs, performs any necessary treatments or procedures, reviews the patients medication profile, develops the initial plan of care including the visit frequency of services needed, obtains the verbal certification and initial orders from the attending physician and the hospice medical director. Obtains agreement on the Plan of care with other members of the hospice interdisciplinary team. Must provide transportation to site of admission, may be exposed to pets and cigarette smoke.The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence based practice and research. Rn competencies are derived from these models and supported by the WellStar Core Competencies. It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected that the individual upholds the voice of the patient, department and system policies and procedures while supporting service excellence goals.QualificationsRequired Minimum Education:

  • Graduate of an accredited school of nursing with AD or BSN preferredRequired Minimum Licensure/Certification:
    • Current license as registered nurse in the State of Georgia,
    • Current BLS card from the American Heart Association on the first day of employment in the position.
    • Hospice nursing certification preferred.
    • CHPN certification preferredRequired Minimum Experience -
