New office coming soon!!! Full-time RN Case Manager Servicing Patients in Gwinnett, Rockdale, and Newton Counties. Seeking a caring and compassionate nurse to give the gift of hospice. As a RN Case Manager not only will you use your skills to manage patient care at bedside care, but you will also holistically provide support to families and caregivers. This position calls for a candidate with excellent written/verbal communication, problem solving and decision making abilities, and experience developing and maintaining rapport with patients and families as well as the ability to work well with an interdisciplinary team.
