RN- Case Manager - Sign on Bonus

Job Description:

The RN Case Manager is responsible to respond to clinical and non-clinical situations and to represent the organization after normal business hours. As a clinician, this RN provides holistic nursing care to terminally ill patients and their families. Nursing care is based on the nursing process which consists of patient/family assessments, diagnosis of key problems, development/management of the plan of care, implementation of interventions, providing patient education and evaluation of patient outcomes. The RN Case Manager has the following responsibilities: Clinical

  • Responds to after hours clinical issues promptly and with clinical proficiency.
  • Contributes to the overall care of patient/families and assures patient care is based on the nursing process and is in compliance with state and federal regulations, TAPESTRY policies, procedures and protocols.
  • Demonstrates knowledge and competence in the multiple facets of quality care of the hospice patient and their family.
  • Demonstrates knowledge of pain management and symptom control as related to terminally ill patients.
  • Ensures that the medical record and plan of care accurately reflect the assessment and care provided.
  • Educates patient/family on physical, emotional and psychosocial aspects of care, the disease process, symptom and pain management, signs and symptoms of death.
  • Acts as a liaison between patient/family and physician and others involved in the team.
  • Actively promotes an open line of communication and a professional relationship with all members of the hospice team, patients, families, facility staff, and physicians.
  • Works with Admissions Coordinator, Director of Clinical Services, Patient Care Manager, or other internal and/or parties in the referral/admission process or in the delivery of patient care.
  • Establishes and directs the CNA Plan of Care when performing admissions.
  • Notifies supervisors/RNCMs of perceived patient care needs.
  • Proficiently documents using the electronic documentation system according to company protocol.
  • Ensures accuracy, thoroughness, and timeliness of documentation in accordance with agency policy and state/federal regulations.
  • Maintains current and accurate patient records.
  • Provides for accurate medication and/or DME profiles to ensure accuracy of patient information and accurate billing.
  • Acquires Benefit of Election authorizations and performs admissions when necessary.
  • Responsible for obtaining verbal or telephone orders for medications from physicians, calling orders into pharmacy, assuring delivery to patient in timely manner and evaluating patient for effectiveness and side effects.
  • Accountable for proper utilization of hospice benefits. Orders DME and medical supplies related to patient needs. Seeks to control costs related to patient's terminal care needs (labs, equipment, supplies, medications, etc.)
  • Responsible for all admissions that are received during on call hours as well as admissions received after 4p.m.
  • Always make an on call visit when a facility calls.
